HIPAA Notice of Privacy Practices (for Patients)

Contents

Effective Date: October 19, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

ATTENTION: If you speak a language other than English, free language assistance services are available to you. Appropriate auxiliary aids and services to provide information in accessible formats are also available free of charge. Call 1-888-890-0690 (TTY: 711).

About This Notice

Frontier Radiology, LLC (“Frontier Radiology,” “we,” “our,” or “us”) is a radiology and teleradiology practice, organized as a Delaware limited liability company, and a Covered Entity under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Our radiologists interpret imaging studies, such as X-rays, CT scans, and MRIs, that your treating provider or hospital orders for you.

This Notice explains how we may use and disclose your protected health information (“PHI”), your rights regarding your PHI, and our legal duties. PHI is information that identifies you and relates to your past, present, or future physical or mental health or condition, the health care provided to you, or payment for that care.

Because we usually provide services at the request of your treating provider, and not directly to you, we generally have an “indirect treatment relationship” with you. We post this Notice on our website and will give you a copy on request.

Harrison.AI Services, Inc. (“Harrison.ai Services”) provides non-clinical management, administrative, operational, and technology services to Frontier Radiology as a business associate. Harrison.ai Services may access PHI only as permitted by its business associate agreement with us and by HIPAA. Harrison.ai Services does not provide medical care.

This Notice applies only to patient health information. Information about website visitors, healthcare professionals, job candidates, and business contacts is covered by our Website and Business Privacy Notice and our Careers Privacy Notice.

Our Duties

We are required by law to:

  • maintain the privacy and security of your PHI
  • give you this Notice of our legal duties and privacy practices
  • follow the terms of the Notice currently in effect
  • notify you if there is a breach of your unsecured PHI.

How We May Use and Disclose Your PHI

Treatment

We may use and disclose your PHI to provide, coordinate, or manage your care. For example, we share our radiology report with the provider or hospital that ordered your study, and we may consult with other physicians involved in your care.

Payment

We generally provide our services under contracts with hospitals and health systems, which bill you or your health plan for imaging services. We may use and disclose your PHI to obtain payment from the hospital or health system that engaged us, and to support that hospital’s or health system’s billing.

Health care operations

We may use and disclose your PHI for our business operations, such as quality review and peer review, training, credentialing, accreditation, licensing, legal and audit services, and business planning. For example, we may review reports to assess and improve the quality of our radiology services.

Business associates

We may share your PHI with business associates that perform services for us, such as Harrison.ai Services, billing companies, and information technology providers. Each business associate must agree in writing to protect your PHI.

Other Uses and Disclosures Permitted or Required by Law

We may use or disclose your PHI without your written authorization in the following situations, subject to the conditions and limits that HIPAA and applicable state law impose:

  • As required by law: when federal, state, or local law requires it.
  • Public health: to report disease, injury, or vital events, to report adverse events or product problems to the Food and Drug Administration, or to report suspected abuse or neglect.
  • Health oversight: to government agencies for audits, investigations, inspections, and licensure.
  • Legal proceedings: in response to a court or administrative order, subpoena, or other lawful process.
  • Law enforcement: for certain law enforcement purposes, such as complying with a warrant or court order.
  • Coroners, medical examiners, and funeral directors: to carry out their duties.
  • Organ and tissue donation: to organizations that handle organ, eye, or tissue procurement.
  • Research: for research approved by an institutional review board or privacy board, or as otherwise permitted by HIPAA, such as with your authorization or using a limited data set under a data use agreement.
  • Serious threat to health or safety: to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
  • Specialized government functions: for military, national security, and protective services, as the law allows.
  • Workers’ compensation: as authorized by workers’ compensation laws.

Uses and Disclosures That Require Your Written Authorization

We will not use or disclose your PHI for marketing purposes, or sell your PHI, without your written authorization. If we ever hold psychotherapy notes, we will not use or disclose them without your authorization, except as HIPAA allows. Other uses and disclosures not described in this Notice will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

We do not use PHI for fundraising.

Substance Use Disorder Records

If we receive records protected by federal confidentiality rules for substance use disorder treatment programs (42 CFR Part 2), we will handle them as those rules require. Those records, or testimony relaying their content, will not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you unless you give written consent or a court orders it after notice and an opportunity to be heard, as required by 42 CFR Part 2.

More Protective State Laws

Some state laws give greater protection to certain types of information, such as HIV/AIDS, mental health, genetic, or substance use disorder information. Where state law is more protective than HIPAA, we follow the more protective law.

Your Rights

You have the following rights regarding your PHI. To exercise any of these rights, contact our Privacy Officer using the details below.

  • Get a copy of your records: You may ask to see or get a paper or electronic copy of your PHI that we maintain. We will respond within the time the law requires and may charge a reasonable, cost-based fee.
  • Ask us to correct your records: You may ask us to amend PHI you believe is incorrect or incomplete. We may deny your request, and if we do, we will tell you why in writing.
  • Request confidential communications: You may ask us to contact you in a specific way or at a specific address. We will accommodate reasonable requests.
  • Ask us to limit what we use or share: You may ask us not to use or share certain PHI for treatment, payment, or operations. We are not required to agree, except that we must agree if you ask us not to share information with your health plan about a service you paid for in full out of pocket, unless the law requires us to share it.
  • Get a list of disclosures: You may ask for an accounting of certain disclosures we made of your PHI in the six years before your request.
  • Get a copy of this Notice: You may ask for a paper copy of this Notice at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you: If you have given someone medical power of attorney, or someone is your legal guardian, that person can exercise your rights.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us by contacting our Privacy Officer, or with the U.S. Department of Health and Human Services Office for Civil Rights at 200 Independence Avenue SW, Washington, D.C. 20201, by calling 1-877-696-6775, or by visiting www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.

Changes to This Notice

We may change this Notice, and the changes will apply to all PHI we maintain. If we make a material change, we will post the revised Notice on our website with a new effective date and make it available on request.
